The Injury Prevention report would typically be completed by the staff member charged with managing your Injury Prevention program. The form provides and overview of the Incident, the Supervisors details, and allow the Injury Prevention manager the opportunity to request an Investigation and request specific Injury Prevention tasks to be assigned by the department Director where the incident took place.
This simple three step process completes the Incident report. The information collected provides your facility with a complete record of the incident should it escalate to a WC claim and more importantly, data that will allow you to develop department specific prevention programs to reduce future incidents and injuries.
